Disease Prevention Programme
Retail Medical Scheme has introduced the Disease Prevention Programme to support members who are at risk of diabetes or cardiovascular disease. If you meet the eligibility criteria and enrol in the Programme, you will receive 12 months of personalised support, including dedicated clinical care, health coaching, and access to additional Scheme benefits. These services are aimed at helping you improve your health and reduce your future health risks.

Together with your Premier Plus GP, and with guidance from a Health Coach and a Dietitian in our network, this programme supports you in actively managing your risk of developing a chronic condition.
You'll receive additional benefits to help you monitor and manage your health, along with coordinated, high‑quality care designed to deliver better health outcomes.
How to join the Disease Prevention Programme?
We identify members who may be at risk of diabetes or cardiovascular disease by reviewing information such as your Health Check assessment results, health claims, family history, and other relevant data.
If you meet the Scheme's eligibility criteria:
- A Health Coach will contact you to explain the Programme and how it can support your health.
- With your consent, your Premier Plus GP can enrol you in the Disease Prevention Programme through HealthID.

What is a Health Check?
A Health Check is a simple but helpful set of basic health screenings and preventive tests which you are covered for annually. These include:
Blood pressure
Blood sugar
Cholesterol or lipogram tests
Weight or body mass index (BMI) assessments
Non-smokers declaration (NSD)

A Health Check helps you take proactive control of your health by giving you valuable insights and support.

The Programme uses an advanced predictive model to identify members who may be eligible. This is based on your Health Check results. If your results show that you are at higher risk of developing diabetes or cardiovascular disease, you may be eligible for the Programme. Eligible members can then be enrolled by their Premier Plus GP.
The goal of the Disease Prevention Programme is to help reduce your risk of developing diabetes. This is done by identifying and addressing risk factors early, such as raised blood glucose levels. Support may include guidance on healthy eating, physical activity, and, where clinically appropriate, prescribed medicine. During the Programme - and at its conclusion - your Premier Plus GP will work with you to determine the most appropriate next steps. This may include continuing lifestyle changes, ongoing medicine, or further assessment. In some cases, a diagnosis of diabetes may be confirmed, allowing you to move forward with the right care and support.
Your Premier Plus GP will help to register you for diabetes on the Chronic Illness Benefit (CIB), giving you access to benefits designed to help you manage your condition effectively.
